How to schedule an insurance covered appointment for the plans listed above (except Cigna):
In order to make an insurance covered appointment, you will first need to visit TLN’s website and provide your insurance information. Make sure “Steele Lactation, LLC” is written in the box “I already have an IBCLC that I’d like to be paired with.” TLN will notify you and Steele Lactation, LLC via email to let you know if your plan is approved or denied for insurance billing. Once you submit your details, TLN will notify both you and Steele Lactation, LLC via email to inform you of your plan’s approval or denial for insurance billing.
If you have already qualified for coverage through TLN with another IBCLC, you will not need to complete the form again. Please simply contact me, and I will request to change in your provider through TLN. Once your plan is accepted in-home visits will incur a $35 travel fee.
If your plan is approved through either TLN or Wildflower, you can return to this website to schedule your appointment. However, if your plan is denied, you can book a self-pay visit on this website.
If you are needing to cancel or reschedule, this must be communicated at least 24 hours before the appointment time. Cancellation with less than 24 hours notice from the scheduled appointment time is subject to a $50 cancellation fee. No cancellation fee will be charged due to illness if the appointment is rescheduled within 10 days. Cancellation fees are not eligible for insurance reimbursement.
